Please note the following:

Form Completion
Remember to complete all sections of your renewal form. To date, 20% of forms have been returned due to incomplete data.

Canada Post
First class mail is considered regular mail service with Canada Post.
If you are NOT including extra documents, the cost of postage for domestic mail should be $1.18.
After mailing their renewal form to the College, some members have had their form 'returned to sender' by Canada Post. Prior to the renewal mailing, Canada Post confirmed CMLTO materials met mailing requirements. However, to avoid any possible inconvenience, we suggest you indicate 'To' and 'From' on the return envelope.

Damaged Envelope
If you have lost or damaged your return envelope, use a regular envelope to mail your renewal form.

Moved?
If you moved after October 13, 2009, or did not advise CMLTO of your change of address prior to October 13, 2009, your 2010 registration renewal form was sent to your old address. Preprinted renewal forms will not be re-issued, so please use one of the blank forms posted below.
