Fees, Receipts and Certificates
The chart below indicates the fees charged by the College of Medical Laboratory Technologists of Ontario (CMLTO) in accordance with the CMLTO By-Law. Please note that fees are subject to change.
Also, please note if you are Indigenous, you may be eligible for GST/HST exemption on applicable CMLTO fees. You may submit a verified copy of a valid certificate or other document to the Registration Department by following the CMLTO Submission Guideline.
Application for registration - non-refundable assessment fee
$150.00 + 13% HST $19.50 = $169.50
Pro-rated annual registration fee
Once an application is approved, the registration fee is pro-rated in the first year of membership only, according to the month in which you became a member.
Practising registration fee: $340 + 13% HST $44.20 = $384.20
Non-Practising registration fee: $170.00 + 13% HST $22.10 = $192.10
Annual registration renewal
Practising Certificate of Registration: $340 + 13% HST $44.20 = $384.20
Non-Practising Certificate of Registration: $170.00 + 13% HST $22.10 = $192.10
The annual registration renewal fee is payable by December 31.
Penalty
If the fee is paid after December 31, and before February 1, a penalty of 20% of the annual fee is applied.
Practising Certificate of Registration: $340 + 13% HST $44.20 +$76.84 = $461.04
Non-Practising Certificate of Registration: $170.00 + 13% HST $22.10 + $38.42= $230.52
If the fee is paid on or after February 1, a penalty of 40% of the annual fee is applied.
Practising Certificate of Registration: $340 + 13% HST $44.20 +$153.68 = $537.88
Non-Practising Certificate of Registration: $170.00 + 13% HST $22.10 + $76.84= $268.94
Reinstatement
Suspended members will pay the Reinstatement fee + Annual Registration Renewal fees of the previous year(s) the member failed to pay+40% penalty+ HST.
Reinstatement fee $100.00 + 13% HST $13.00 = $113.00
Other fees
Letter of Standing & Verification of Professional Licensure / Visa Screen – USA fees:
$44.25 + 13% HST $5.75 = $50.00 (members)
$132.74 + 13% HST $17.26 = $150.00 (former members)

Reprint income tax receipts prior to 2012: $14.28 + 13% HST $1.86 = $16.14
Reprint of wall certificate: $25.00 +13% HST $3.25 = $28.25
NSF/credit card declined: $14.28 + 13% HST $1.86 = $16.14
